1. Web Submission:  This is the quickest and easiest method to submit your application.  Go to our On-Line Course Application and enter your information as requested.  Upon submission you will receive immediate e-mail notification that your application has been received along with a cover sheet to submit with the required materials.
   
   
2. Download Application (PDF): Download a PDF version of the Course Application and type or handwrite in black ink the necessary information.  Mail the application along with the required materials to the address below.
   
   
3. Request an Application:  Call or e-mail Carol Hamel, Admissions Coordinator, (508) 289-7401; and ask to have an application mailed or faxed to you.  Mail the completed application along with the required materials to the address below.
